PATIENT RESOURCES

Everything you need to get started.

Whether you are a new patient preparing for your first visit, an established patient needing forms, or someone in crisis right now- this is where to find what you need.

01 · FOR NEW PATIENTS

What to expect at your first visit

Your first appointment is a 60 to 90 minute video session focused on understanding your story. We will ask careful questions, listen more than we talk, and leave you with a clear sense of next steps.

  • Plan for 90 minutes, though we may finish earlier.
  • Find a private space with reliable internet.
  • Have your insurance card and any current medication list handy.
  • You do not need to come prepared with a diagnosis – that is our job.
02 · BEFORE YOUR VISIT

Telehealth setup guide

All LumenMind appointments are conducted via secure HIPAA-compliant video. A few minutes of setup ahead of time means our session can focus on what matters.

  • Use a laptop or tablet when possible — phone screens work but are tighter.
  • Test your camera and microphone before the visit.
  • Connect from a private room where you will not be overheard.
  • Click the secure video link sent by email 10 minutes before the appointment.
03 · PAPERWORK

Forms and documents

New patient intake forms are sent securely after booking. There is nothing to download or fax. If you need an additional document, reach out and we will send what you need.

04 · EXTERNAL
