At LumenMind, ongoing support means established patients have a real relationship with their provider, predictable follow-up cadences, and access to the practice when something urgent comes up between visits.

It also means the care plan is allowed to evolve. What you needed two years ago may not be what you need now. We adjust without making you start over.

BEFORE

Where things stand

Between visits, we ask you to track a few simple things: how you are feeling, sleep, anything notable that has happened. A few seconds in a form, not a journal assignment.
DURING

The check-in

We meet for 30 to 45 minutes. Less of an interrogation than a conversation. We look at what is working, what is not, and what to adjust if anything.
AFTER

Quiet adjustments

Most ongoing visits do not result in big changes. They keep care on track. When a real adjustment is needed, we make it deliberately, with a written plan.
FOLLOW-UP

Whenever needed

Most established patients see us every two to three months once stable. If life shifts, we shift the cadence. If something is urgent between visits, the practice is reachable.

How often do established patients see you?

Once stable, every two to three months is typical. More often when something is changing, less often when things are steady.

For non-urgent questions, the patient portal is the best route. For urgent concerns, we have a process and you can reach the practice during business hours.
We provide care under Pennsylvania licensure. If you move, we can continue temporarily while you find a new provider, and we help coordinate the handoff.

We start with an evaluation in nearly all cases. It is how we understand the full picture and how you decide if we are a fit.

Yes, and we treat it with the same care as taking someone on. Tapering off care thoughtfully is part of what we do.
